Address Change
If you have recently moved and would like to formally change your address in our school’s system please complete an Address Change form and submit it to our front office.
Residency Affidavit
If a parent and child are living in a household where they do not have any proof of residency under their name, they may supply the school with a notarized Residency Affidavit and supply proof of residency under the name listed on the affidavit.
Phone Number Change
Please update your phone number with the front office anytime that it changes.
Forms can be obtained from the front office.
